How penile traction therapy works
Penile traction therapy applies a steady, comfortable stretch along the length of the shaft. The SizeGenetics™ device delivers straight, one-direction longitudinal traction — gentle, single-axis tension held over extended wear, never angled or directional. Sustained mechanical load is thought to stimulate mechanotransduction, the cellular process by which tissue converts mechanical force into a biological adaptation response, the same principle applied in orthopedic and reconstructive medicine. Chung & Brock (Therapeutic Advances in Urology, 2013) documented that mechanical traction drives "reorganization and remodelling of collagen fibres into uniform densely packed fibrils parallel to the axis of mechanical strain." Worn consistently over several months at roughly 4–6 hours per day, the device is designed to support gradual changes in penile length and girth. What the clinical research shows about penile traction
Penile traction therapy is the subject of more than 15 peer-reviewed clinical publications. The therapy's strongest documented outcomes cover two areas: curvature correction in men with Peyronie's disease, and length recovery after prostatectomy. Peer-reviewed data on length gains in non-surgical settings exist but come from smaller studies in specific clinical populations — not the general healthy male population.
Two cohort studies reported length outcomes in non-surgical settings. Gontero and colleagues (Journal of Sexual Medicine, 2009, PMID 19138361) enrolled 15 men with stabilized Peyronie's disease wearing a penile traction device at least 5 hours per day; at 6 months, mean stretched penile length increased by 1.3 cm as a secondary outcome (the primary endpoint was curvature reduction). Nikoobakht and colleagues (Journal of Sexual Medicine, 2011, PMID 20102448) studied 23 men presenting with clinically reduced penile length, using a different traction device for three months; mean gains of 1.7 cm were recorded in both flaccid and stretched states (P<0.05). Both studies used specific clinical populations and different devices; they represent the best available non-surgical proxy data for how traction may affect penile length.
The most rigorous randomized evidence comes from a post-surgical context. Toussi and colleagues published a single-center randomized controlled trial in the Journal of Urology (2021, PMID 34060339) enrolling 82 men after radical prostatectomy, using the RestoreX penile traction device at 30–90 minutes per day. At 6 months, the traction group preserved and recovered significantly more penile length than controls — +1.6 cm versus +0.3 cm (p<0.01). This study documents post-surgical length recovery after prostatectomy, not enlargement in healthy men; 87% of participants said they would repeat the therapy and 93% would recommend it to others. Across the traction literature, reported adverse events have been mild and temporary, with no serious adverse events reported. A note on girth: the measured outcomes across the penile traction therapy literature are predominantly length gains — the cited studies tracked length, and the axial collagen realignment that Chung & Brock described aligns tissue along the line of pull. Thank you so much to the designers of this device, and also to the customer service team, you're all GREAT. My customer service guy, Arnold, he gave me perfect advice as to how to keep the glans in the device without choking it too hard, that was paramount in me even being able to wear the device indefinitely.
I do remove the device every 20 minutes of wearing, as the glans will get purple after 20 minutes of wearing, and I take 5 minute breaks. The rosy colour of the glans comes back instantly after releasing the silicone tube.
I use the no-slip black rubber mat directly onto the penis, on top of it the sponge sleeve and then the silicone tube, fully onto the sponge sleeve. In this setting, the glans doesn't slip out of the silicone tube anymore, stays in, always.

This is actually one of the greatest inventions ever created. It gives guys an opportunity to add an extra few inches to their manhood. I got the Ultimate system, shipping came in a few days. Just starting using the device, there are no instructions given with the package, just a QR code to some video links. After watching them there are some things that are not explained very well, some parts and their uses, not explained at all.
The biggest thing is when tightening the strap, it's very easy to pinch one's skin on it. This is very painful so I'd highly recommend the comfort pad. These foam pads look like they would get worn down over time so I recommend getting extra. Even with the ultimate set they only give one. And they charge $20 for a set of 4 which is a bit of a rip off as I'm sure the foam is probably only a few cents but it's the right size. The more frugal out there could probably do something equivalate with a large foam pad and some scissors for under a dollar.
Second issue for me at least was getting it tight enough that it wouldn't slip off. The video explains to have the extension bars equal to the length of your flaccid penis but I'm a grower not a shower so my flaccid length is only 2 inches and the minimum length for the basic extension bars is 3 inches so I had to do some stretching and pulling while fully compressing the extension bars to get it on. This took me well over an hour of trying different things to get it to work.
Third issue, the strap for me even at the tightest setting, while it would grip my foreskin, my shaft would simply slide out as the extension bars tried to pull it so with the comfort pad I had to take a couple pieces of tissue fold them to add some additionally padding and girth so that the strap would be tight enough so that my penis would not slip out.
After all that though, it worked very nicely.
The videos recommend for the first week to do 1 hour sessions at 1200 grams of tension, this is contrary to others tension therapies I've seen that recommend 4-6 hour sessions at 600-800 grams to start off. So I just went with what I felt was a good tension and see how long I could comfortably maintain it, about a 1000g of tension. After about an hour with the strap super tight so it would stay, the head of my penis started getting cold and blue from lack of blood circulation so I took it off to take breaks and alternated like this after it warmed up a bit. None of this is explained in any of the videos so to side effect or things to watch out for or what to expect other than gains and benefits.
I'm currently taking a break as the ligaments around my groin feel very stretched like I did the splits, waiting for my body to adapt and adjust. I know use of this device is a marathon not a sprit so patience is needed.
It's only been a few days so obviously I don't have any measurable results yet but what is obvious is that the device can stretch out the area and over time this should lead to extra length so I'm hopeful as there is a body of medical studies about extension therapy working. But I treat it like a workout and include rest as my body signals for me to do so.

1st Month for those on the fence It's been a month and I am pleased with the results have been amazing, My routine only varies on the days that I work I try to do it for at least 6hrs and my days off at least 10hrs, for those that have peryones(curvature) the first thing you will notice is increase in girth that's probably due to the lack of muscle tissue that has not grown typically on the side where it curves, you'll notice how the trunk or shaft of your johnson is the first thing that gets thick and strong which will already prove to work wonders when you get to action time, so after 1 month of use it seems like my girth has increased significantly and my length probably has close to half inch, but the most glorious aspect of all of this is that my wife cannot stop screaming when I'm inside of her(I swear this is 100 percent truth, for those that believe in God you all know swearing is a big deal), I also made her cry (lol ) which was a bit confusing at first cause I thought she was going through pain but she kept kissing me and told me afterwards that those were tears of joy and that she is amazed of how strong she feels me down there.My main purpose with this review is to address those who are on the fence about getting this device specially those with Peryones: - The size, girth and strength of your johnson is in a crippled state because one side(typically the side where your johnson is curved towards) of it has not developed muscle tissue to be in its optimal state, hence it means that because of it you have a reduced girth and and length just by having Peryones WHICH IS NOT HOW IT WAS MEANT TO BE, believe me is the equivalent of having a muscle imbalance, so far this is the best way to fix that muscular imbalance on your johnson. - It might sting a little bit: For those that stretch regularly or do yoga you will understand how muscle tears work , in my case with Peryones it stings on the side where the tissue has not grown or developed and I have to take deep breaths or change my breathing patterns according to the gravity of the sting(it literallyfeelsas if needles are getting pulled out of the side where you have the Peryones), if the stings get too overwhelming I remove the device and I let it rest until the stinging stops to put it back on and complete my hrs , if you do yoga or stretch regularly you won't have a problem reading when to take it off and putting it back on, but just remember because you have peryones your experience will not be pain free ,Everytime you put on the device probably around the 3rd or 4th hr of wearing the device it will literally feel as if needles are being pulled out of the side where you have peryones, just be patient and breath accordingly cause it I'll be worth the effort. DO NOT HYPER EXTEND YOUR JOHNSON, GO SLOW WITH THE EXTENSION AND INCREASE LITTLE BY LITTLE. - If you you feel uncomfortable telling your partner about the device just tell her the following in your own words(for those with Peryones): Say you magically stumbled upon information that led you into deep research and you finally found out that you have an unnatural muscuĺar imbalance(which is not a lie) on your Johnson and that you are going to get it fixed with a device, say something that goes along those lines and she won't give you issues, in fact she'll love you more once she starts feeling you stronger, longer and girthier(typically after 1st month of use). In closing: feel happy that you found a way to give your Johnson a workout thats effective and that can be worn anywhere without being detected, my curvature has not yet improved too significantly but my length and specially my girth has, I guess once the muscle tissue becomes more balanced that's when Mr Johnson is gonna start getting straighter, at least it feels that way so far. It definitely works, but be patient and consistent to see results
I’ve been using it for about six months and gained around one inch so far, wearing it 5-6 days a week for around 3-5 hours a day. I keep track of my hours and measurements in a spreadsheet (so far 400 hours in) and take photos of measurements to remind myself how much progress I’ve made.
The size increase is definitely noticeable and has been a boost to my confidence. I’ve seen some increase in girth too. It also makes you harder and for longer since it stimulates blood flow, so in that sense I plan to continue using it even after reaching my length goal. I wish I discovered the device years ago.
The device itself is very well-made with good quality material as I haven’t had any issues with it or parts failing, etc. It still looks brand new even after a lot of use. It’s comfortable to wear too but takes some getting used to at first with putting it on and securing a good grip (a bit of talc powder helps too). I find it's best to wear if you're laying down.
I’ve also been impressed with the customer support I’ve received, having had to email a few times along the way with questions and asking for advice, and they always reply promptly and helpfully. The same person replies to my emails, so it’s nice having consistency since they are familiar with me as a customer.
My advice/tips for new users - follow the start-up programme, be patient and consistent, clean the device daily, keep track of your hours and take regular measurements. I use the silicone tube as a strap and find that every few months or so I need to order a new one since it will lose its tightness overtime due to stretching, but it’s inexpensive to buy.

In my experience, the Sizegenetics stretching device is comfortable, but only if you know how to make it comfortable. I am uncut, so not any other stretching device works for me; I have tried vacuum devices and those have been the worst for me because my foreskin gets in the way. With this stretcher device, I use the silicon strap and the protection pad and those work for me. In the past, I have wore this device for 400 hrs and it did elongated my penis a little bit, but when I stopped wearing the device, I lost those gains. My journey with size genetics started a few years ago after I had recovered from a minor surgery where I was put to sleep, and the surgeon attempted to catheterize me and couldn’t get the Catheter in because of stricture. He used a surgical rod to open the passageway through my penis and unknowingly tore up my Urethra, causing severe scar tissue. After months of healing and contraction of inner tissue, I had lost almost half of an inch in penis length.
A urologist friend recommended the Size Genetics device to me. After using the device for several months, not in a very disciplined manner, I had barely gained back 3/8 of an inch. I have been dealing with Peyronie's disease for several years. I didn't want to go for surgery, I thought there must be another solution. The design and engineering of this product is excellent. I have been using it for more than one year now, and I find the silicone tube noose fit me best. One tip would be to clean the tube every day with hot water and dishwasher soap, this will result in higher friction (more grip) which again makes it more comfortable. Before I did this the tube tended to slip slightly and that was uncomfortable.
